Innovating for Adolescent Self-Management

What Makes Our Project Unique?

This project was born from a shared observation made during our nursing training: many adolescents living with asthma face significant challenges in adhering to care and gradually taking ownership of their health.

Rather than focusing solely on the disease itself, we chose to explore the adolescent experience, the factors influencing care adherence, and the role nurses can play in supporting the transition toward greater independence and self-management.

Our work also addressed a practical gap identified during our research: the lack of concrete tools to help nurses support adolescents in their journey toward autonomy. We therefore transformed scientific evidence into practical, evidence-based resources designed to support nurses, adolescents, and families in everyday care.

The benefits of our tools

Our three tools were designed to address some of the most common challenges in adolescent asthma care, including poor treatment adherence, limited understanding of the disease, difficulties in developing self-management skills, and the often poorly structured transition from pediatric to adult healthcare services.

As poor adherence can lead to asthma exacerbations, and reduced quality of life, these tools aim to strengthen knowledge, and preparedness for transition.

By encouraging active participation in care and decision-making, they empower adolescents to become true partners in managing their own health rather than passive recipients of treatment.

The impact of our project

If our project succeeds, the most meaningful change will be a shift in how adolescents living with asthma are supported throughout their healthcare journey.

Rather than being viewed solely as recipients of care, adolescents will be recognized as active partners in the management of their health.
Through tailored support, therapeutic education, and progressive involvement in decision-making, they will be better equipped to understand their condition, manage their treatment, and prepare for the transition to adult healthcare services.

At the same time, healthcare professionals will have access to practical, evidence-based tools that help them provide more structured, personalized, and adolescent-centered care.

Ultimately, our ambition is to contribute to a culture of care that empowers adolescents, strengthens nurse–patient partnerships, and supports a smoother transition toward lifelong self-management.

Our project aims to transform adolescents with asthma from passive recipients of care into confident and active partners in their own health journey.
